Moscow completes transformation of oncology service

Work on a large-scale renovation of the infrastructure for providing oncological care in Moscow is entering the finish line. Mayor of the capital Sergey Sobyanin announced the completion of the construction of a new medical and diagnostic complex of the Moscow Clinical Scientific Center named after A.S. Loginova. It is symbolic that this happened on the eve of the opening of the Moscow Cancer Forum.

As part of the new medical and diagnostic complex MKNTs them. A.S. Loginov Center for outpatient care, a radiotherapy department, 18 state-of-the-art operating rooms, a laboratory and much more. Here they will treat the most complex localizations of malignant tumors and perform all types of operations for cancer of the head, neck, thyroid gland, respiratory tract, lungs, esophagus, stomach, reproductive system, pancreas, liver, skin.

And also perform organ transplantation for malignant neoplasms. For the first time, a department of high-precision radioisotope diagnostics — PET/CT, SPECT/CT — will start working here. The capabilities of the Center will allow providing comprehensive medical care to the most severe — comorbid patients, that is, those who have several more severe chronic diseases.

Stages of a long journey

In recent years, other major changes have taken place in the capital's oncological service system. “This medical area was the first where we implemented new approaches to organizing care,” said Anastasia Rakova, Deputy Mayor of Moscow for Social Development. – We have implemented client pathways for the fastest way for a patient – from suspected disease to treatment and follow-up. We formed the framework of the city's oncology service, reconstructed the largest facilities and built the country's most powerful oncology center. Loginova».

Since 2019, a new standard of cancer care has been introduced in Moscow, providing for an individual client path for each localization and duration of the disease, and depending on the complexity of the case. Before the opening of the Moscow Cancer Forum, Minister of Health of the Russian Federation Mikhail Murashko noted that Moscow medicine today is the locomotive of the healthcare system. These are new technologies, digital solutions, and the introduction of artificial intelligence programs aimed at early detection of oncology and accompanying a patient with such a serious illness.

As part of the new standard of cancer care, anchor cancer centers were created in the capital, combining inpatient and outpatient care, the level of equipment of which meets the advanced world standards. Such centers were organized on the basis of the largest city hospitals — MKNTs them. A.S. Loginova, MGOB No. 62, GKB im. S.P. Botkin, MMCC «Kommunarka», GKOB No. 1. And today all stages of treatment — from examination to dispensary observation — take place in one institution.

The structure of the anchor centers also included pathomorphological laboratories, whose capabilities were seriously increased. To equip them, the city purchased more than 300 units of modern equipment, which made it possible, among other things, to automate the process of processing diagnostic biomaterial as much as possible. A pilot project for the digital transformation of pathological research continues. Doctors create digital copies of biomaterials and a single database of histological images.

Surgical departments are equipped with equipment for the least traumatic minimally invasive operations — laparoscopic or robotic. Patients who need radiation therapy can receive it free of charge not only in city hospitals, but also in private Moscow clinics. Increasing the city and its own capacity. Thus, in the new building of Kommunarka, the construction of a center for 3D and 4D radiation therapy that meets high international standards is being completed. And the patients of the Moscow Scientific Center named after A.S. Loginova, these progressive technologies are already available.

Other oncological infrastructure of the city is also being actively updated. Thus, a modern laboratory of oncomorphology and molecular genetics has been created at the Moscow Oncological Hospital No. 62. A new oncosurgical building has been opened at Oncological Hospital No. 1. New CAOPs are currently working in the City Clinical Oncological Hospital No. 1, GKB named after I. Botkin and at the Kommunarka Medical Center.

Ahead of schedule

Moscow became the first region in Russia to switch to new federal clinical guidelines for the treatment of the most severe diseases ahead of schedule — 3 years ahead of schedule. common types of cancer.

Thanks to the re-equipment of diagnostic units, the availability and efficiency of all types of examinations have significantly increased. The city has a sufficient number of facilities for radiation diagnostics — CT, MRI, X-ray machines and expert-class ultrasound, radioisotope diagnostics (PET-CT and SPECT-CT) are available. And today, on average, about a week passes from the moment the diagnosis is confirmed to the start of treatment in the capital.

Cancer drug treatment, including targeted and immune therapies, is provided at the expense of the city budget or within the framework of a regional benefit. The capital's drug therapy program, which is part of the basic CHI program, covers 90% of patients who are prescribed the most effective drugs that allow targeted action on the tumor and avoid many side effects.

It is also important to note that thanks to the digital platform of Moscow healthcare all information about the patient is loaded into his electronic card, which allows specialists to see all the information about the patient throughout his treatment.

The Moscow Cancer Society operates in the capital, uniting the efforts of specialists in their desire to improve the quality of oncological care using the best world practices. Thus, last year, at the initiative of the Moscow Oncological Society, observational studies were launched aimed at forming a clinical base for the use of the latest oncological drugs. 12 oncological drugs are being tested in four nosologies: lung, kidney, breast and prostate cancer.

Digital services in oncology

Telemedicine technologies are actively used in the capital at various stages of cancer care. Digital models of diseases of different localizations and stages, computer «vision» have been created. At the Digital Oncology section, experts discussed the practical implementation of developments in the field of digital pathomorphology, telemedicine, as well as technologies based on artificial intelligence in radiation diagnostics. At the same time, the Moscow oncology service is actively interested in the digital experience of colleagues from other countries and regions of Russia. So, at the session «Digital services in the profile» Oncology and oncohematology «and the help of volunteers», the experience of Spain was presented.

New treatments for breast cancer

In modern medicine, despite the availability of clinical guidelines and constantly updated clinical research data, there are many complex and ambiguous cases where there is no single correct answer, and the same problem can be viewed in different ways. At the session “Innovations in the treatment of breast cancer. Analysis of complex cases” showed the work of a multidisciplinary team in discussing such cases. And also considered the optimal way of interaction between team members, including a surgeon, oncologist, morphologist and radiotherapist.

Treatment and rehabilitation of patients with cancer of the head and neck

Experts call head and neck tumors the most dramatic section of modern oncology. One of the urgent problems of their treatment is the full restoration of organs after surgery. Without adequate reconstruction of removed or resected organs, it is impossible to maintain the quality of life, so many patients refuse radical treatment. The Expert Council «Controversial issues of surgical treatment and rehabilitation of patients with head and neck cancer: old problems — new solutions» discussed controversial issues of surgical rehabilitation of patients with head and neck tumors in order to expand the possibilities of surgical treatment. Palliative care in Moscow

Oncology and palliative care seem to be separate worlds: in one they are fighting for life, in the other they are accompanying them at its end. But in the world of the patient, they are two banks of the same river. And it depends on the doctor how the patient will get from one to another: without fear and physical suffering, or choking with anxiety and hopelessness. At the session “Oncology + Palliative Care = Patient’s Quality of Life”, they talked about what is important for every Moscow oncologist to know: how the structure of palliative care in Moscow is arranged, how to route a patient on time and correctly to receive it, and how to conduct a “severe” treatment with a patient in an environmentally friendly way. conversation.

Side effects of anticancer therapy

Modern anticancer therapy and a variety of adverse reactions against the background of patient pathology require the creation of a team of various specialists to effectively deal with complications. As part of the discussion “A multidisciplinary approach to toxicity management and rehabilitation of cancer patients receiving anticancer therapy”, experts analyzed the features of managing patients receiving anticancer treatment aimed at reducing the risk of a number of adverse events in the short and long term.

Global approach to drug therapy

In order for the latest advances in medicine to bring maximum benefit to patients, it is important for a modern doctor to constantly update his knowledge, keep abreast of all the latest trends, formulate clinical questions and hypotheses.

ASCO Annual Meeting is the largest global oncology conference of the year and one one of the most anticipated events in the oncology community – brings together 40,000 oncologists from around the world to educate colleagues and the world about the most advanced treatment options, discuss approaches to treatment and prevention. The session “Post ASCO Drug Therapy – Clinical-Changing Research” was dedicated to the most interesting reports presented at the last ASCO conference. These reports can already change approaches to the diagnosis and treatment of cancer today.
